HPV Vaccinations

Dear Parents/Carers


The Human Papilloma Virus (HPV) vaccination is part of the NHS school-based immunisation programme in the UK. In previous years the vaccine has been offered to girls in Year 8 to protect them against cervical and other HPV related cancers. In line with government recommendations as of September 2019 the programme will now include all boys in Year 8 too. This is because the evidence is clear that the HPV vaccine helps protect both boys and girls from HPV-related cancers.

The HPV vaccine consists of two injections given into the upper arm at an interval of between six and twenty-four months.  The first immunisation will be given on Friday 7th February and you will be notified when the second dose is due. If your child moves to a school outside Oxfordshire before receiving their second dose please contact the School Health Nurse Immunisation Service in your new area for advice to ensure the course is completed.

Consent to vaccinate your child is now via an online consent form. We will no longer be sending out paper consent forms, so it is important that you complete the form online so that your child is able to receive their vaccination. To access the online consent form, please click on

You will need to enter the unique code for your child’s school. The code is OX123294 Please submit your form by Wednesday 05/02/2020, 2.00pm to ensure your child receives their vaccination. We are unable to accept forms submitted after this date. If you experience any problems accessing the form, please telephone your School Health Nurse Office (details overleaf) who will be happy to help.

Important parent/carer information – Please read the leaflet available here which explains the vaccination programme. If you require a hard copy, please contact your School Health Nurse Office. It is important that you read this leaflet carefully so that you are fully informed to provide consent for your child to receive the vaccination.

If your child is unable to receive his or her immunisation due to absence or for another reason, a catch-up session will be arranged in school and you will be advised of this date.

Declining the offer of HPV vaccination

If you decide you do not want your child to receive the HPV Vaccination, please complete and submit the consent form. If you would like to share why you do not want your child to receive the HPV vaccine, it will help us plan and improve the service.


Withdrawing Consent

If you have consented for your child to be vaccinated and subsequently wish to withdraw this consent please inform your School Health Nurse Office by telephone and by e mail.
