HPV Online Consent

I am writing to let you know of an important change to our Immunisation Programme. We will no longer be sending out paper consent forms for all our school based immunisation programmes including, our Human Papilloma Virus (HPV) Programme for Year 8 Students or the Tetanus/Diphtheria/Polio and Meningitis ACWY programmes in Year 9, instead there will be an online process for submitting Consent.
HPV Year 9 girls – If you completed a paper consent form for your child’s HPV Dose 1 vaccination in Year 8 you will not need to submit an electronic form for their HPV Dose 2 this year.
Prior to the session in school you will receive an email from your child’s school, with all the information required to complete the new process for giving on line electronic consent. This can be accessed and completed on a Smart Phone, Laptop/desktop computer or iPad.
You will receive, in the email:
- The link to the electronic consent form.
- The school code which enables you to access the electronic consent form.
- The date the immunisation will take place in school.
- The date the electronic consent form must be submitted. If the form is not submitted before the closing date your child will not be able to be immunised at the school session and will have to book into a catch up session, which may not be at your child’s school.
